Your Medical Record

Confidentiality & Your Medical Records

The practice complies the new General Data Protection Regulations (May 2018) - see attached GDPR Notice.  There is also a hard copy on the noticeboard in the Waiting Room, if you would like a copy please ask at Reception.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

SystmOne (our computer system)

SystmOne is used by various other Health Service providers eg. Podiatry, Community Nursing, Physio, Child Health.

Using the Government guidelines SystmOne have introduced a policy for sharing of data from patients’ medical records.

We need all our patients to complete a form to register their preferences.

If you say Yes (Consent) to this, it means your medical record is ‘pooled’ so that any other service that is involved with your care can access this ‘pooled’ record.

This is called a ‘share out’

If you say No (Dissent) your GP medical record will only be visible to The Riverside Practice.

When you are subsequently seen by another service eg. Physio, they will also ask if you consent to their part of the medical record being seen by other services, which will include your GP Surgery. This is called a ‘Share in’

You can change these choices at any time by letting the relevant practice or service know.

Summary Care Records

The Riverside Practice is contractually obliged to upload patient details from our clinical system to the Summary Care Record (SCR) Database held by the NHS.    These uploads will be done automatically.

The Summary Care Record will contain basic information about any allergies you may have, your medication you take, and any unexpected reactions to drugs etc.  The intention is to help clinicians in Hospitals, Accident & Emergency Departments and "Out of Hours" health services to give you safe, timely and effective treatment.

You will always be asked before your Summary Care Record is accessed by healthcare staff (unless you are unable to give your consent e.g. you are unconscious) you will always have the right to refuse to share information.

This upload is different to the Care Information Programme from NHS Digital (see details below).  There are ways to opt out if you feel that you do not wish to participate, and we will respect your wishes at all times.
